B)<span class="elsevierStyleHsp" style=""></span>Eritema inguinal bilateral, bordes bien definidos, no fisuración.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><p id="par0005" class="elsevierStylePara elsevierViewall">Un hombre de 82<span class="elsevierStyleHsp" style=""></span>años, con antecedentes de dislipemia, diabetes mellitus y cardiopatía isquémica crónica, presentaba un exantema macular rosado, no descamativo, pruriginoso, sin despegamiento epidérmico, de varias semanas de evolución, de inicio en las ingles y con una extensión posterior a los flancos y la espalda. El paciente había cambiado de marca comercial de esomeprazol hacía un mes. Ante la sospecha de una toxicodermia, se retiró el esomeprazol y se trató tópicamente con crema de metilprednisolona aceponato 1<span class="elsevierStyleHsp" style=""></span>mg/g, con una resolución de las lesiones en una semana. En la analítica destacaba una eosinofilia leve y en la biopsia cutánea se observó un infiltrado inflamatorio perivascular superficial de predominio linfocítico con un edema leve, una discreta ectasia vascular y ocasionales eosinófilos (<a class="elsevierStyleCrossRef" href="#fig0005">fig. 1</a>A). Se realizó prick test con ambas marcas de esomeprazol y una prueba de provocación, con resultados negativos. Tras reiniciar su tratamiento con esomeprazol, no presentó una reaparición de las lesiones.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall">A los 5 meses consultó nuevamente por una recidiva de las lesiones (<a class="elsevierStyleCrossRef" href="#fig0005">fig. 1</a>B). Destacaba la administración de la tercera dosis de la vacuna COVID-19 (VC19) Comirnaty 48 horas antes del inicio de los síntomas. Reinterrogando al paciente, el episodio previo se había desarrollado a las 72<span class="elsevierStyleHsp" style=""></span>horas de haber recibido la segunda dosis de la misma vacuna.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Ante la sospecha de un exantema intertriginoso y flexural simétrico por fármacos (<span class="elsevierStyleItalic">symmetric drug-related intetriginous and flexural exantema</span> [SDRIFE]), asociado a la administración de Comirnaty, se completó el prick test con la vacuna Comirnaty y con polietilenglicol (PEG) y se realizaron pruebas epicutáneas (PE) con batería estándar del GEIDAC, metales, metales especiales de Martí Tor®, cosméticos, acrilatos y PEG, siendo todos los resultados negativos. No se realizó prueba de provocación con la vacuna dada la reciente administración de la tercera dosis. Recibió tratamiento tópico con una crema de clobetasol 0,5<span class="elsevierStyleHsp" style=""></span>mg/g, con una resolución de las lesiones a las 4<span class="elsevierStyleHsp" style=""></span>semanas.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Basándonos en las características clínicas y anatomopatológicas de las lesiones, la relación temporal con la administración de la vacuna, la ausencia de otras causas que pudieran justificar el cuadro y teniendo en cuenta los criterios de causalidad de Naranjo<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">1</span></a>, el paciente fue diagnosticado de un «probable» SDRIFE inducido por la vacuna Comirnaty.</p><p id="par0025" class="elsevierStylePara elsevierViewall">El SDRIFE, antes conocido como síndrome de Baboon, es una forma de dermatitis de contacto sistémica. Se manifiesta como un exantema eritematoso, simétrico, localizado preferentemente en las nalgas, en los genitales y en las zonas de flexión de las extremidades. Ocurre cuando un individuo está sensibilizado a un alérgeno de contacto al que se expone por vía sistémica (transcutánea, transmucosa, intravenosa, intramuscular, oral, inhalatoria). Es una entidad benigna, sin manifestaciones de toxicidad sistémica, con buen pronóstico tras la retirada del agente causal<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">2</span></a>.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Se han publicado 10 casos de SDRIFE en relación con la VC19, cinco casos por Comirnaty<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">3-6</span></a>, dos por CoronaVac<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">6,7</span></a> y tres por Vaxzevria/ChAdOx1 nCoV-19 (AstraZeneca-Oxford)<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">8-10</span></a> (<a class="elsevierStyleCrossRef" href="#tbl0005">tabla 1</a>). Los síntomas pueden presentarse desde las primeras horas de la exposición hasta varias semanas después<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">2</span></a>. En siete pacientes el exantema se manifestó tras la segunda dosis de la vacuna. En todos los casos los síntomas y signos de afectación sistémica estuvieron ausentes.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0035" class="elsevierStylePara elsevierViewall">Los hallazgos histopatológicos del SDRIFE son inespecíficos<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">3,4,7</span></a>. Podemos encontrar infiltrados linfohistiocitarios perivasculares superficiales que pueden incluir neutrófilos o eosinófilos<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">2</span></a>. La espongiosis puede o no estar presente.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Previamente se han descrito casos de SDRIFE asociados a vacunas que contenían tiomersal entre sus componentes. Este componente no forma parte de las VC19 descritas. El adyuvante de la vacuna Comirnaty estudiado como posible responsable es el PEG.</p><p id="par0045" class="elsevierStylePara elsevierViewall">En los casos descritos por la VC19 se realizaron PE en cuatro pacientes<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">3,6,10</span></a>, con un resultado positivo para el níquel en uno de ellos<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">3</span></a>. El prick test se realizó en tres pacientes, con un resultado negativo<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">6,10</span></a>. Las PE y el prick test pueden dar resultados negativos. Esto podría deberse a la menor absorción de los fármacos por vía sistémica cuando son aplicados a través de la piel. El test intradérmico con la vacuna fue positivo en un caso; sin embargo, esta respuesta puede ser positiva, como una respuesta inmune local en pacientes que han recibido previamente la vacuna<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">6</span></a>. Las pruebas de provocación suelen ser positivas en la mayoría de los casos de SDRIFE<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">2</span></a>. No se han descrito pruebas de provocación con las VC19. Todos presentaron una buena respuesta a los corticoides tópicos u orales, con resolución de las lesiones cutáneas a las pocas semanas.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Tras iniciar la vacunación frente a la COVID-19 a nivel mundial se están describiendo casos de SDRIFE secundarios a estas vacunas.
